Health Information Request
To request medical records of past hospital stays or surgeries in a Methodist Healthcare facility and to ensure compliance with the Health Insurance Portability and Accountability Act (HIPAA) privacy regulations, the following form must be completed thoroughly.
The form may be completed online, but must be printed, signed by the patient and mailed or delivered to the hospital or surgery facility where the procedure was performed.
The address of the facility may be found by "clicking" on Methodist Facilities. If you have any questions pertaining to the requested Medical Records, please call the Medical Records department of the facility.
